// What you receiveWhat lands on your desk

Treatment is where comp claims are supported or undercut. Every referral comes with a documentation trail your office can actually use — and a person to call when you need something.

  1. 01

    C-4 initial report, on time

Filed with the Workers' Compensation Board within 48 hours of your client's first visit, with a thorough history and a clear opinion on causation.

  2. 02

    Progress reports on schedule

Follow-up reports filed at the intervals the Board requires, so benefits and authorizations aren't held up by missing paperwork.

  3. 03

Disability & work status, every visit

Degree of disability and work restrictions documented consistently — the details that drive your client's indemnity benefits.

  4. 04

    Treatment requests handled

When care falls outside the Medical Treatment Guidelines, requests are prepared and submitted with the supporting findings.

  5. 05

    Narratives when you need them

Narrative reports for hearings, depositions, and settlement conferences. Tell us the date and we work to it.

Your clients are covered too: multilingual intake, scheduling around shift work, appointment reminders, and offices reachable by subway and bus — so they show up and stay in treatment.

// Questions from firmsStraight answers

Will my client owe anything for treatment?

Care related to an accepted workers' compensation claim is billed to the carrier, not the injured worker. We verify coverage at intake and flag anything unusual before the first visit.

Can you see a client whose claim is contested?

Send the referral anyway. We'll review the details with your office and talk through options before scheduling.

How fast will you reach my client?

Our intake team contacts your client promptly after the referral comes in and confirms back to your office once the first visit is booked.

How do I get records or a narrative?

Contact your point of contact or note it on the referral form. For narratives, include the hearing or deposition date.

What languages do you support?

Select your client's preferred language on the referral form. Intake is arranged in that language with bilingual staff or an interpreter.

Where are you located?

[Office locations to be added by client.] Every location is chosen to be reachable by public transit.

Can my paralegals send referrals?

Yes. Anyone at your firm can use the referral form — list the attorney of record and the best contact for updates.
